From my limited knowledge, I believe other ways to achieve that problem would be a cracked head, lack of lubrication, deposits on valve stem (rust, carbon, crude) bad spring, guide, rotator and bad luck.

How to fix, I would bring head to a reputable shop and let them do their stuff. Pressure test, check spring rate, etc.

I would expect others to have better answers but thought I'd guve my 2Cents.
