Thru Hull Removal

How does one go about securing the outboard mushroom head of a thru hull raw water intake so I can spin off the inboard seacock? I am replacing my rotted backer blocks and am having a difficult time getting everthing loose. I notice two vertical tabs inside but have not yet figured out what tool to use to grip. Thanks for the answer in advance.
